Hx Medical History Taking 2.0

This application was designed to aid medical students in digitally recording patients' Medical History (Hx) using a structured and systematic approach. It offers a comprehensive set of questions for each phase of the Medical History record, streamlining the data entry process by allowing students to input patient responses directly into the patient's profile. Once a patient's profile is completed, it is stored on the user's device alongside other entered profiles. Users can generate either a concise medical report or a detailed one for any stored profile at their convenience. After generating the report, users have the option to review it, email it, or print it on a compatible wireless printer.

This app assists in diagnosing the illness as an initial step towards treatment through gathering essential information like primary complaints, current issues, duration of symptoms, onset of disease, symptom persistence, complaint frequency, disease progression, influencing factors, medical history, past illnesses, surgeries undergone, regular medications, family history, professional history, social status, and social level. The output includes a summarized assessment of the patient's Medical History as well as a comprehensive medical report on the case. It enables sending reports via storage or email and provides the option to print the patient's name along with the medical student's name on the report.
